you buy or renew by fax or mail and provide an email address, your Confirmation of Insurance will be emailed to you.
Otherwise, your Confirmation of Insurance will be sent to you via U.S. mail.

Q3: I need coverage to begin as soon as possible. Is it possible for my Study USA-HealthCare policy to go into effect the
day I enroll?
A:
No, however, your policy can go into effect as soon as one day after you enroll online.

Q4: Are a student's dependents eligible for coverage?
A:
Yes. Spouse and/or children (under age 18) of enrolled students can be enrolled for insurance with the student's initial
enrollment, or within 31 days of birth, legal adoption, marriage or arrival in country of study. If your dependents are not
eligible for Study USA-HealthCare, they can enroll in one of our
other plans for people outside their home country.

Q5: My school requires an Insurance Compliance Form or a Waiver Form be completed as proof of my insurance. Can
you complete this form for me? What should I do?
A:
First, purchase Study USA HeathCare Insurance. After your purchase (enrollment) is complete, simply fax your school's
Compliance or Waiver Form to us at (610) 537-9828. We will complete it and fax it back to your school for you. Forms received
by 12:00 noon Pacific Time will be faxed back to your school by 5:00 p.m. Pacific Time, the same day.

Q6: Can I buy Study USA-HealthCare if I am already studying outside my home country?
A:

Q7: Can I renew my coverage?
A:
Yes, as long as you fulfill the eligibility requirements in Question 1 above.

8. How do I renew my coverage?
A:

Those who originally enrolled by mail or fax and did not provide an email address, can renew online or by using a print form
that will be mailed to you one month prior to your current coverage Expiration Date.

Coverage Clarification
Q10: Can I go to any doctor or hospital, or must I use my school's Student Health Center or a preferred provider from
Study USA-HealthCare's network?
A:
For International Students Studying in the U.S.:
  1. If you choose to be seen at your Student Health Center when it is equipped to treat your condition, you will receive the
in-network benefit and your deductible for such incidents will be waived.
  2. For other incidents, if you use a doctor or hospital in Study USA-HealthCare's large preferred provider network,
MultiPlan, Inc., you will pay a lower deductible than if you go outside the network.

For U.S. Registered Students Studying Outside the U.S.: Outside the U.S., Study USA-HealthCare does not have a Preferred
Provider Network.
  1. You can choose to be seen at the Student Health Center or at another doctor or hospital and receive the in-network
    benefit.
  2. If you go to the Student Health Center when it is equipped to treat your condition, your deductible for such incidents
    will be waived.
  3. You can also use any doctor or hospital. The Emergency Assistance Company can help you locate a doctor or hospital in
the country where you are studying.

Q11: Will my deductible be waived if my school has no Student Health Center?
A:
Yes, as long as medical services are received from a MultiPlan Preferred Provider Network member.

Q12: Does Study USA-HealthCare cover Pre-Existing Conditions?
A:
Pre-Existing Conditions are defined by the policy as "an injury, illness or condition which was contracted or which first
manifested itself, or for which a licensed physician was consulted; or for which treatment or medication was prescribed,
within 6 months prior to the effective date of the Covered Person's coverage under this Program. Pre-Existing Conditions
shall be excluded from coverage for a period of 6 months following the effective date of coverage under this program. A
Covered Person must be continuously insured." See Study USA-HealthCare's complete list of
policy exclusions in the plan
details section.

Q13: Does Study USA-HealthCare cover pregnancy?
A:
Yes, it covers the pregnancy of an insured student or enrolled spouse if the pregnancy begins during the coverage period.
It does not cover the pregnancy of a dependent child.

Q14: Will my Study USA-HealthCare policy cover me if I travel outside the U.S.?
A:
Yes, Study USA-HealthCare covers the Insured worldwide, except in their country of permanent residence, where they will
only be covered up to $1,000 for illness and injuries incurred within the first 30 days of entering their country of permanent
residence and as long as they fulfill the requirements in Question 1 above.

For International Students In The U.S.
Q15: Is a network provider located in my area?
A:
Most likely. For a complete listing of MultiPlan Network participants please contact MultiPlan, Inc. at 800-557-6794 or
MultiPlan, Inc.

  1. Click the SEARCH THE NETWORKS button
  2. Click on the MultiPlan PPO logo
  3. Then perform your search by choosing Find a Healthcare Facility or Find a HealthCare Practitioner.

Q16: How will I receive care at In-Network level if there are no Network providers in my area?
A:
If there is no MultiPlan provider qualified to provide the care needed within a 50 mile radius of your residence, you can
choose to be treated outside the Network by a non-MultiPlan provider. In this situation, benefits will be paid at in-network
level.

Q17: After graduation, I plan to remain in the U.S. for practical training. Am I still eligible for coverage under Study
USA-HealthCare?
A:
No. Once you graduate and are no longer attending school, you are no longer eligible for Study USA-HealthCare. You
