- 90/10 Co-insurance in US and Canada
If you visit with in PPO network then after deductible, you are responsible to pay 10% and plan pays 90% of covered
medical expenses of next $5,000. Then, plan pays 100% of covered medical expenses up to policy maximum.
If you visit outside PPO Network then after deductible, you are responsible to pay 20% and plan pays 80% of covered
medical expenses of next $5,000. Then, plan pays 100% of covered medical expenses up to policy maximum.
- No Co-insurance outside US and Canada
After deductible, plan pays all the covered medical expenses up to policy maximum.
For USA, PPO Network is provided by "First Health Network". If you go within PPO network then provider will do direct
billing with Insurance company. Then Insurance company will send you the bill later to you to get deductible, co-
insurance...etc. You can view this network online.
Outside USA, this plan also have PPO network internationally and you can view that network online.
- Network Negotiated Fee (NNF) for medical treatment within PPO Network
When you go within PPO network, you will be charged Network Negotiated Fee (NNF) medical treatments because
those provider are under the contract with "First Health Network". This fee is also known as discounted fee in many
cases.
- Prescription Drug Discount Card
This is a discount saving program available to every certificate/policy holder of "Patriot Travel" and "Patriot Go Travel"
plans. This program allow card members to purchase prescriptions at one of over 35,000 participating pharmacies in US
and receive the lower of 1) Universal Rx contract price or 2) The pharmacy regular retail price. This always assures the

If Patriot Travel is purchased for minimum of three months, coverage may be renewed (without break in coverage) for
a total of up to 2 years. Renewals are available in whole month or daily increments and may be completed online or by
using a paper application.
- No renewal fee (if you renew more than a month)
If you renew plan for more than a month, then there will be no renewal fee.
If you renew plan for less than a month, then there will be $5 renewal fee. However, renewals of less than one month
are available only online.
